Department Overview
The Hemostasis and Thrombosis Center (HTC) is home to over 45 faculty and staff, engaged in the clinical, research, public health, outreach and education missions. The HTC is a multidisciplinary center, providing comprehensive care to adult, adolescent and pediatric patients.
This position is responsible for providing the services of a licensed pharmacist within the HTC. The pharmacist specializes in caring for patients of all ages with hemophilia and other bleeding disorders in an ambulatory/outpatient setting. The Triage Pharmacist serves as the primary clinical pharmacist within the HTC, actively participating in patient visits, providing direct patient care, and collaborating closely with prescribers and the interdisciplinary clinic team. This role focuses on medication therapy management, ensuring that prescribed treatments are appropriately coordinated, dispensed, and optimized to meet the needs of each patient.
Additionally, the Triage Pharmacist plays a key role in patient education, offering counseling on treatment regimens, self-administration techniques, and adherence strategies. While the primary focus is on clinical care, the pharmacist is also responsible for supporting the medication dispensing process as needed to ensure seamless continuity of care for HTC patients. The position supports the mission, initiatives, and goals of the HTC clinic, ensuring alignment with its commitment to comprehensive patient care.
Function/Duties of Position
- Identifies, addresses, and documents complex therapeutic concerns related to the treatment of hemophilia and other bleeding disorders. Ensures appropriate selection and use of factor replacement therapies, non-factor therapies, and adjunctive treatments. Collaborates with prescribers to optimize medication regimens in accordance with evidence-based guidelines and individual patient needs.
- Reviews and evaluates prescriber’s orders for accuracy, completeness, and clinical appropriateness, ensuring alignment with established treatment protocols. Coordinates dispensing plans to align with treatment regimens determined by HTC providers, ensuring timely and appropriate medication access. Supports the medication dispensing process when needed to ensure continuity of care.
- Provides comprehensive medication education to patients and caregivers, ensuring understanding of treatment goals, administration techniques, dosing, side effects, storage and handling, and adherence strategies. Offers individualized counseling for patients initiating or transitioning therapies.
- Monitors therapeutic response and adverse effects of drug therapy. Consults with practitioners when questions or problems are identified. Appropriately reports all adverse drug reactions, drug defects, and medication errors.
- Supports collaborative treatment planning by providing input on product selection and other relevant considerations to help optimize treatment adherence and patient outcomes.
- Researches and evaluates professional literature to respond to clinical inquiries, providing up-to-date drug information to HTC providers, clinic staff, and patients.
- Assists with developing and operationalizing programs to ensure the proper use of medications and supports continuous quality improvement initiatives within the clinic. Provides support for special projects as assigned by the supervisor or clinic, and may have opportunities to travel for outreach clinics, conferences, and/or regional meetings.
- Participates in on-call responsibilities as needed. Provides after-hours support on a rotational basis to address urgent medication-related issues and assist patients with critical needs outside of standard clinic hours.
Required Qualifications
- B.S. Pharmacy or Pharm.D.
- Community pharmacy practice experience or inpatient pharmacy practice experience.
- Pharmaceutical care principles. Disease related modification of drug therapy.
- Project management skills and experience in contemporary medication use systems and electronic medical record systems.
- Strong interpersonal communication skills.
- Systematic problem solving skills.
- Ability to work independently with minimal supervision.
- Competency in patient interviewing and teaching skills.
- Excellent knowledge of third party payors, insurance billing, and prior authorization guidelines of specialty medication.
- Working knowledge and skills using MS Word, Outlook, Excel, PowerPoint, and Access programs.
- Successfully completes initial and continued competencies as required for maintenance of standard of practice, legal regulatory compliance and OHSU policies.
- State of Oregon Board of Pharmacy Pharmacist licensure.
Preferred Qualifications
- Advanced pharmacy practice residency, BCPS or 5 years of equivalent experience.
- Experience in specialty pharmacy practice, including hemophilia/bleeding disorders either inpatient or outpatient.
- Inpatient hospital pharmacy practice.
- Three years of health system experience.
- Experience in managing hemophilia patients in an ambulatory setting.
- Experience with factor product prescriptions, labeling and documentation in CareTend and Epic.
- Specialist Certifications (BCPS, BCOPS, etc.).
- State of Oregon Pharmacy Preceptor licensure.
